This quarter, the NIC will bring members together to discuss S4S at the health system and national levels.

The American Society of Health-System Pharmacists (ASHP) Standardize 4 Safety (S4S) initiative highlights the importance of standardized concentrations of intravenous medications across the continuum of care, however aligning with this initiative can be challenging.  Additionally pharmacy practice continues to evolve and thus standard concentrations selected by S4S are routinely evaluated and updated. 

Joining us will be Jeff Hurren, PharmD, BCPS, Medication Safety Officer at Michigan Medicine, and Mary Ann Kliethermes, B.S., Pharm., Pharm.D., FAPhA, FCIOM, Director, Medication Safety and Quality at ASHP.
